Chronic tricuspid regurgitation: integrated severity (ASE 2017)

Version 2026.10.1

Asks whether most (more than half) of the 7 specific criteria for mild or for severe are met, then uses vena contracta width with PISA EROA and RVol for intermediate cases (three grades). The four-grade TR subclassification is not validated and TVARC five-tier grading is a separate framework.

Flowchart:
Applicability
  • Native valve with chronic TR. Prosthetic and repaired valves, and acute regurgitation, are outside the algorithm.
  • TR is graded by integrating specific signs and measurements; no single parameter produces a grade. Poor image quality or discordant parameters give an indeterminate result.
  • Quantitative parameters are used only when valid for the study (holosystolic timing, jet geometry, no shunt or second regurgitant valve for stroke-volume methods, hemispheric PISA).
  1. Scope of the algorithm
Is this a native valve with chronic TR?
State whether the valve is native and the regurgitation chronic.
