RV dimensions interpreter (basal, mid, longitudinal, RVOT, wall thickness)Classifies each RV linear dimension against the 2025 Table 1 partitions, with the view and timing each measurement requires; indexes the basal diameter when BSA is known.
RV fractional area changeFAC = 100 × (RVEDA − RVESA)/RVEDA from the RV-focused apical four-chamber view; normal >35%.
TAPSE interpreter with acquisition checksClassifies TAPSE (cm or mm) against Table 1 after checking cursor alignment, method and the clinical contexts in which TAPSE under- or overestimates RV function.
RV S′ interpreter with acquisition checksClassifies tissue Doppler S′ (normal >9.5 cm/s) after checking Doppler alignment (<20°) and contexts of under- or overestimation.
Integrated RV systolic assessmentReconciles longitudinal (TAPSE, S′, free-wall strain) and global (FAC, 3D RVEF) indices against the Table 1 normal limits and states the limitations of each.
RA size: dimensions and areaClassifies RA major dimension, minor dimension and end-systolic area against the 2025 right-heart reference partitions.

Native valves

LVOT stroke volume, SVi, cardiac output and indexCircular LVOT area × LVOT VTI gives stroke volume; indexed to BSA and multiplied by heart rate for CO and CI.
Peak gradient (simplified Bernoulli) with proximal correctionΔP = 4V²; include the proximal velocity when it exceeds 1.5 m/s or the jet is below 3.0 m/s.
Continuity-equation AVA, indexed AVA, VTI ratio and velocity ratioAVA = LVOT CSA × LVOT VTI / AV VTI, with AVA/BSA, the dimensionless VTI ratio, and the peak-velocity ratio.
PISA effective regurgitant orifice areaEROA = 2π r² × Va / peak regurgitant velocity, with the radius and the CW peak velocity taken at the same moment.
PISA regurgitant volumeRVol = EROA × regurgitant-jet VTI (CW), with timing limits: not valid by simple multiplication when regurgitation covers only part of its period.
MR volumetric regurgitant volume and fraction (LV total SV − aortic forward SV)RVol = LV total stroke volume (EDV − ESV) − aortic forward stroke volume; RF = RVol / LV total SV; optional EROA = RVol / CW jet VTI.
Regurgitant fraction with an explicit denominatorRF = RVol / total stroke volume through the regurgitant valve × 100; the method (and therefore the denominator) must be chosen.
Vena contracta width: valve-specific interpretationAccepts the measured vena contracta width and grades it against the valve-specific table, with the technique and limitations of each valve.
AR jet width / LVOT widthCentral AR jet width divided by LVOT width at the same site, × 100; valid only for central jets.
Mitral valve area by pressure half-time (native rheumatic MS)MVA = 220 / PHT. Eligibility-gated: not within 24 h of balloon valvuloplasty, not with moderate/severe AR, not in degenerative (MAC) MS, not for prosthetic valves.
Mitral valve area by planimetry (2D or 3D)Accepts a traced orifice area and checks that it was measured in a short-axis plane at the leaflet tips in mid-diastole; classifies against the rheumatic MS partition.
Integrated MS parameters (area, gradient, PHT, PASP, heart rate)Classifies each Table 1 parameter, checks the heart rate at which the gradient was measured, and reports agreement or discordance without voting.
Pulmonic stenosis gradient and severityPeak gradient 4V² from the CW transpulmonary velocity, proximal correction when the RVOT velocity is not negligible, Table 11 severity, and the PASP = RVSP − PV gradient check.

Prosthetic and repaired valves

Prosthetic mitral valve: EOA and VTI ratioContinuity-equation EOA (stroke volume / prosthetic MV VTI) and VTI ratio (VTI_PrMV / VTI_LVOT). Never derived from pressure half-time.
Prosthesis-patient mismatch: measured indexed EOAMeasured EOA / BSA graded with the position- and BMI-specific cutoffs (Table 7). Applies only when the prosthesis is functioning normally.
Prosthetic aortic valve: change from baseline and hemodynamic criterion for deteriorationCompares current mean gradient, EOA and DVI with the designated baseline (stable-condition TTE, 1–3 months after the procedure). Reports whether the Table 6 hemodynamic criterion is met; it does not diagnose structural valve deterioration.
Prosthetic mitral valve: Table 11 parameter panelCounts how many of the assessed Doppler parameters fall in the normal, possible-stenosis and significant-stenosis ranges of Table 11. Diagnostic accuracy is best when most parameters agree.
