Segmental wall motion response to stress: normal, ischemic, fixed and viability patterns

Version 2026.10.1

Interprets one segment's response from resting function, the low-dose/low-workload stage and peak stress, using the response definitions of the 2020 ischemic stress document. Patterns describe segments; the overall study interpretation integrates all segments, the global response, adequacy of stress and ECG.

Applicability
  • Each segment is graded at rest and with stress; assessment of wall thickening (not just motion) is required, and a biphasic response should be sought with dobutamine or bicycle stress when resting abnormalities are present.
  • Interpretation of a stress study must integrate the extent, severity and location of abnormalities with the adequacy of stress; inadequate stress reduces sensitivity.
  1. Rest stage
How does the segment move and thicken at rest?
Resting segmental function is required before the stress response can be interpreted.
